The move to secondary school is a huge step for both child and carer. This chart helps prepare children facing that move, become responsible for themselves, their belongings and assists with the daily preparation expected of them upon reaching secondary school.

The activities include: Preparing school bag, preparing school uniform, getting up on time, taking care of myself, eating sensibly, doing some exercise, being polite, doing my homework, listening and acting responsibly, keeping my bedroom tidy and being helpful. The chart also supports and gives guidance to the parents and carers detailing how important their role is during this time.

On the chart children have the responsibility of monitoring and recording their achievements. Each achievement earned goes towards simple rewards.

Each pack contains:
	A3 chart (420mm wide x 297mm high).
	A supportive information sheet with
	working example, tips and guidelines.
	4 x sticky fixing pads. 1 x dry wipe pen.
